We participate with most HMO's, PPO's, and commercial insurance plans. We also accept Medicaid, Medicare, and Tricare. Other major insurance plans that we accept is Blue Cross/Blue Shield, Aetna , and United Health. Please check with your insurance company to see if we are on your plan. As a courtesy, we file claims on behalf of the patient. Our Tax ID # is 75-6064033.
Please be aware of your insurance plan's rules regarding treatment and payment. Many plans require a co-payment by the patient, which will be your obligation to pay at the time of service. You are personally responsible for the payment of services provided to you unless you are covered by Medicaid or specific contract plans. Having insurance does not release you of this responsibility.
If we must send you to a specialist and a referral is required, we will obtain this for you. If you make a follow-up appointment to the specialist, it is your responsibility to notify us that another referral is needed. We require 7 working days to obtain the follow-up referral.
